Favicon in report viewer?

I understand this may be Off Topic for this board, but has anyone figured out how to add a Favicon to the Report Viewer?

Fatal Exception in SQL Server

Hello,
We got a SQL Server (no SP) running on Win 2k SP4. Recently been getting
this error message in event viewer and at the same time the program that
access SQL Server crashes.
Error: 0, Severity: 19, State: 0
SqlDumpExceptionHandler: Process 64 generated fatal exception c0000005
EXCEPTION_ACCESS_VIOLATION. SQL Server is terminating this process.
Does anyone have any idea what it is please. Thanks

If this is SQL2000, apply SQL2000 sevice pack 4 + AWE memory hotfix first.
If this SQL2005, open a support case with Microsoft. But I'd strongly
suggest that you run SQL2005 on Windows 2003 SP1.

But you will need to log a case with Microsoft CSS , because the error you
posted is not sufficient enough , we need at least the stack output from the
AV , and if there was any mini dump created in the log folder during this
time...
So as linchi said please log a case with Microsoft CSS for speedy resolution

Fatal error

Dear All,
I got a WIndows 2000 Server running MSDE. Recently it keep crashing with
this error in event viewer:
Error: 0, Severity: 19, State: 0
SqlDumpExceptionHandler: Process 58 generated fatal exception c0000005
EXCEPTION_ACCESS_VIOLATION. SQL Server is terminating this process.
I have updated it to SP4 still the same problem. Does anyone know
anything about this error message please

Have you checked the windows event log for any messages that may indicate an
issue, also it is probably worth running CHKDSK to make sure that there is no
corruption on the disc drives.

The error message is from Events Viewer. I check everything else at
around the same time nothing special. I had scheduled a CHKDSK to run
yesterday with no issues found. Any other idea please?

Without knowing what was actually happening on the server at the time, it is
almost impossible to say what has caused this error. There are many issues
where the message may have been generated http://tinyurl.com/7kglf but I have
not found one that was not cured prior to SP4. You may want to raise a call
with PSS if this persists

>|||I am getting the same error
SqlDumpExceptionHandler: Process 58 generated fatal exception running sQL
2000 sp4 and latest windows 2000 OS updates.
But I am also running in a clustered environment.
One of my servers never crashes (with pretty much the same patches etc.)
while the other server continues to crash.
I starting to think it has something to do with the actual machine hardware
and not the software.

Failure Audit - Event ID: 18456

Hi I'm using SQL Server 2005 and ,
I keep getting the following message in the Event Viewer:
Event Type: Failure Audit
Event Source: MSSQLSERVER
Event Category: (4)
Event ID: 18456
Date: 1/21/2008
Time: 8:32:00 AM
User: domain\sqladmin
Computer: TCLG-MOSS-DB
Description:
Login failed for user 'domain\sqladmin'. [CLIENT: <local machine>]
Can someone please shed some light on this......

ShelleyYou have audit login set either to "Failed logins only" or "Both failed and
successful logins". You can change the setting vai SQL Server 2005 Management
Studio. Right click on the server instance and pick Properties. Select
Security, and you'll see the audit options. You need to restart the SQL
instance for the audit level cahnge to take effect.

This error basically happens with unsuccessful attempts to log on to the
server?
